Complimentary Phone Consultation:
I am available for a 15 minute initial phone consultation to provide you with information about my services and determine if I am the right therapist for your needs. Intake and ongoing sessions are 45-50 minutes in length.
Fees and Payment:
Full session fees and co-pays are due at time of services. Payments can be made via cash, check, Venmo, Apple Pay or other arrangement made between us in advance. I do accept some insurance and for those that I don’t, I can provide you with an invoice for reimbursement from your insurance and/or HSA plan.
Paperwork:
New clients: if you are able, please download the first 4 documents, complete, scan and email them back to me. If you do not have the ability to scan, please print, fill out and bring forms to your first appointment. The additional Release of Information Consent form should also be completed if you are seeing a Psychiatrist or other practitioner with whom I may need to communicate. Thank you.
